CANCELLATION FEE

  • $50 Cancellation Fee

    Port Fairy Osteopathy is a very busy clinic with limited appointments. If for some reason you need to reschedule your appointment, I require 24 hours notice; otherwise you will be charged $50 for the missed appointment. This is in consideration for others and allows me to schedule in patients from my waiting list.

  • Subsequent short-notice cancellations will be charged at the full consultation fee

  • Patients with outstanding accounts will not be re-booked until payment for previous treatment and/or cancellation fees have been paid in full.

PRIVATE HEALTH FUND REBATES

  • Private Health Fund Rebates under “extras” cover can be processed on the day through the HICAPS system.

THIRD PARTIES

  • Port Fairy Osteopathy does not accept Medicare/EPC/CDM plans, WorkCover, TAC or DVA claims.

  • Port Fairy Osteopathy does not invoice to third parties (including aged care).

  • The patient is responsible for full payment of the consultation fee at the time of treatment .

HICAPS/EFTPOS

  • HICAPS and eftpos/credit card facilities available.

Your Personal Health Information and your Medical Record may be collected, used and disclosed for the following reasons:

  • For communicating relevant information with other treating osteopaths, doctors, specialists or allied health professionals

  • For appointment reminder/recall notices

  • Accounting/Medicare/Health Insurance procedures

  • For disease notification as required by law (eg infectious diseases)

  • -For use by all osteopaths in this group practice when consulting with you

  • For legal related disclosure as required by a court of law (eg. subpoena, court order, suspected

    child abuse)

  • For research purposes (de-identified, meaning you are not able to be identified from the

    information given)
