Port Fairy Osteopathy is a very busy clinic with limited appointments. If for some reason you need to reschedule your appointment, I require 24 hours notice; otherwise you will be charged $50 for the missed appointment. This is in consideration for others and allows me to schedule in patients from my waiting list.

Subsequent short-notice cancellations will be charged at the full consultation fee
Patients with outstanding accounts will not be re-booked until payment for previous treatment and/or cancellation fees have been paid in full.
PRIVATE HEALTH FUND REBATES
Private Health Fund Rebates under “extras” cover can be processed on the day through the HICAPS system.
THIRD PARTIES
Port Fairy Osteopathy does not accept Medicare/EPC/CDM plans, WorkCover, TAC or DVA claims.
Port Fairy Osteopathy does not invoice to third parties (including aged care).
The patient is responsible for full payment of the consultation fee at the time of treatment .
HICAPS/EFTPOS
HICAPS and eftpos/credit card facilities available.

For communicating relevant information with other treating osteopaths, doctors, specialists or allied health professionals
